About Us

Our professional staff, and the independent physicians and consultants at The Courtyards at Pasadena are all committed to working with our patients and residents as partners to meet their individual health care, rehabilitation and coordination needs. As care givers, we want to know our patients as individuals, taking care of their health needs, but also considering the person as a whole.

The Courtyards at Pasadena is proud of its history in the community and we treasure our relationships with residents and families. We strive to maximize and enhance the quality of life of our residents in a compassionate and professional environment. The need for long-term care, skilled nursing, or rehabilitation may not occur under ideal circumstances. A fall, stroke, or brief illness can deprive us of our customary feeling of independence and security. Fortunately, we have a team of trained professionals that will assist in developing individualized plans of care, in order to achieve desired goals.

We strive to maintain the highest standard of care to help our residents get back into the community or to create a healthy long term living environment.

In our Rehabilitation Program, the clinical team will evaluate each resident’s unique circumstance to develop and meet individualized goals. During treatments, therapists will work toward improving independence, strength and confidence.

Long-Term Care assists those who may benefit from 24- hour nursing availability and assistance with meals, medications or other activities of daily living. The environment at The Courtyards at Pasadena is designed so that residents may live comfortably, as a part of a community.

Physicians

We understand choosing a physician is one of the most important health care decisions a person must make. Each physician with privileges to practice at The Courtyards at Pasadena has been credentialed and reviewed by our Medical Director Quality Assurance Committee. Although we do not employ physicians we have partnered with a variety of specialist to meet the needs of our patients.

The Medical Director will work with the patient's Primary Care Physician (PCP) to promote continuity of care. We have an open panel for PCPs and welcome them to follow patient care. If your PCP is interested in following your care please have them contact our admissions team to find out more information about the credentialing process.
